How to Access a Patient's Previous Episode of Care in Tridiuum ONE

The following training document will go over how to access a patient's previous episode of care.  If a patient has been out of active treatment for a year, or if a Call Center screener was recently administered and the patient had to call the call center again, the screener will be skipped. In both of these situations, a new episode of care will be created for the patient. To access assessment results for previous episodes of care, just follow the below steps.


  1. Open a web browser on any device with an internet connection and type in: polestarapp.com/admin/login
  2. Enter your Username and Password and select LOGIN.                                                                                  
  3. Select the PATIENTS tab to view a list of your patients.                                                                                 
  4. Select the patient's name from the list to open their Patient Profile Page.                                                             
  5. On the right side of their Patient Profile Page, next to EPISODE OF CARE select the box that says Current and switch it to any of the previous episodes of care listed to view the assessment results from the corresponding episode.                                                                                                             
  6. From this point, you can follow normal protocol to view the assessment results for the episode of care you select.
