How to Locate a Patient and Reroute Assessment from Tridiuum ONE

This training document will walk you through how to locate a patient on the Clinical Home and then how to reroute your patient assessment from the Tridiuum ONE website.

1. Click to open filter options on Clinical Home.

   Open

   

2. Select Provider’s name currently linked to Patient and click APPLY.

   Open

   

3. Once Patient is located, click on Patient’s name to access Patient Profile Page.

4. Scroll to the bottom of the screen to the ASSESSMENTS tab to see the assessment. Locate and click the green pencil.

5. Use drop down menu, select your name, and click SAVE.
