How Find a Patient's Phone Number and Address on Tridiuum ONE

The following training document will show you how to access the Patient Profile Page which displays the patient contact and demographic information. If you use Tridiuum ONE Connect, you can view instructions on how to find that information here.

 

  1. After you login to https://polestarapp.com, find the patient whose information you would like to access. Click on their name to open the Patient Profile Page.

  2. Click View Patient Info to view the patients contact and demographic information.

  3. From this screen you can view patient Date of Birth, patient Phone number(s), and demographics. Click on the Address tab to view their address.

This information comes directly from the integrated customer’s electronic health record system. Any information that displays on this screen is the correct, most recent information. If you work for an organization that is not integrated, please review your electronic health record system to receive your patient information.
