How to Find Patient Contact Information in Tridiuum ONE

This training document walks you through how to find your patient's address and phone number on the Tridiuum ONE web application.


  1. Open a web browser on any device with an internet connection and type in: polestarapp.com/admin/login. 
  2. Enter your Username and Password and select LOGIN.                                
  3. Select the PATIENTS tab to view a list of your patients.                              
  4. Select the pencil icon next to the patient whose demographic information you would like to view. 
  5. The EDIT PATIENT box will appear, including the patient's phone number. Select the Address tab to view the patient's address.
  6. Once Address tab is selected, the address will appear on the screen. Click the X in the upper right to close the window.
