Adding Assessments

Select providers that use Tridiuum via an iframe/SSO that have assessments automatically initiated and shouldn’t have access to manually add an assessment. To compensate for this scenario, we have added a permission to hide instances of adding an assessment outside of an appointment.

Display Add Assessment Permission

  • Created a new permission via Manage Roles: “Display Add Assessment”

    • Permitted (default)

    • Not Permitted

    • The new permission has been added to the Manage Roles CSV export

    • Existing subscriptions will be set to Permitted (default) on the Onboarding Form

    • All existing roles will be set to Permitted (default)

  • When the setting is “Not Permitted”, it hides all instances of manually adding an assessment (“+ ADD ASSESSMENT” and Add Assessment Icons)

Adult Analytics Dashboard

CSV Report

We have updated the scores, alerts, response text, response values to include a wider array of information applicable for MHv4, SUv7, SUv8 into the CSV Export file. This has also resulted in removing unnecessary or confusing data points.

Please Note: Permission to access the Adult Analytics Dashboard and the updated CSV Export is dependent on the organization's configuration. For organizations that only use one assessment, this may result in several blank fields because those fields are not applicable to their assessment.

Primary Care v4

We have released a new standard version of the Primary Care assessment (version 4). Tridiuum’s newest Primary Care assessment utilizes standard mental health measures, plus 10 Social Determinants of Health questions (SDOH) to using the AHC Screening Tool (10 items).

"The Centers for Medicare & Medicaid Services (CMS) Center for Medicare and Medicaid Innovation (CMMI) made the Accountable Health Communities (AHC) Health-Related Social Needs (HRSN) Screening Tool to use in the AHC Model. The Tool can help providers find out patients’ needs in these 5 core domains that community services can help with:

- Housing Instability

- Food Insecurity

- Transportation Problems

- Utility Help Needs

- Interpersonal Safety”

Assessment

The following questions are all asked on the Intake assessment. On the update assessment, the patient will be asked a question to determine which questions are asked:

  1. What is your living situation today?

  • I have a steady place to live

  • I have a place to live today, but I am worried about losing it in the future

  • I do not have a steady place to live (I am temporarily staying with others, in a hotel, in a shelter, living outside on the street, on a beach, in a car, abandoned building, bus or train station, or in a park)

2. Think about the place you live. Do you have problems with any of the following? CHOOSE ALL THAT APPLY 

  • Pests such as bugs, ants, or mice

  • Mold

  • Lead paint or pipes

  • Lack of heat

  • Oven or stove not working

  • Smoke detectors missing or not working

  • Water leaks

Data Sharing Between Organizations

The Patient-Sharing/Data Sharing Between Organizations (DSBO) initiative relies on the idea that all data shared via webhooks or API calls be processed successfully:

  • Created the transaction management layer for transactions, and to ensure the data is reliably created and that the related data is referenced properly for organizations sharing data (DSBO) to alert success or failure of the call.

  • Created ability to access practitioners shared from another Tridiuum ONE organization

Digital Adoption

Tridiuum University Curriculum

We are introducing a new module to the Resource Center called Tridiuum University. This will provide clinicians with a personalized curriculum to learn more about administering assessments, navigating Tridiuum ONE, and analyzing treatment results.

  • Screen and Refer Dashboard
    We are working to clarify the patient information provided on our screen and refer dashboard. While we perfect this, we have added information icons to provide clarification about scheduling appointments, administering assessments, and making referrals. Stay tuned for a further redesign of this dashboard.

  • Patient Contact Information
    We’ve also added new information icons for clinicians to better understand the process of updating patient contact info in the Notifications tab versus the Information tab. Updating a patient’s email address or phone number in the Notifications tab will not automatically update their contact info in the Information tab, while adding new contact information under Information tab modal windows (Add Assessment/Create Appointment) will automatically update their contact information in the Notifications tab.

Mindful Minute Fix

We successfully addressed an issue in which the Mindful Minute: Move guide was cut off and prevented Safari and Chrome users from advancing the module.

Security and Compliance

We continue to make improvements regarding security, performance, and compliance:

  • Migrated the pdi repository to a new directory and consolidated cron jobs.

  • Merged changes from iem-master and master-demo branches to ensure we're using a single codebase

  • As part of our quest to automate installs and upgrades, we have removed random top-level symlinks in the polestar directory and replace them with proper configuration variables:

    • Back up the config files in etc (such as etc/local_env.yml).

    • Delete the polestar directory.

    • Redeploy the polestar directory from the tarball.

    • Redeploy the previously-backed-up config files in etc (such as etc/local_env.yml)

  • As part of our effort on creating better tooling around Tridiuum ONE, we have Included a dot file to specify the desired version file that the application expects to have available so that tools can simply cat that file and install the proper version for:

    • Node

    • NVM

    • RVM

Tridiuum ONE Connect/Call Center

Permissions

We have added 2 new permissions via Manage Roles and to the CSV Export to be able to schedule or refer a patient without a BHI.

Please Note: Using these permissions will not be able to take advantage of the FICOM matching algorithm because a BHI is not present, so the list of recommended providers may not be as optimally matched as patients that have a BHI.

  • Scheduling without BHI

    • when permitted, a connect user can select Scheduling Override Approved even if that patient doesn't have a BHI

    • reports for the screener leverage the same conventions in the BHI sections that state "BHI was not generated as part of this screening encounter"

    • a comment is required before the user can proceed

  • Referral without BHI

    • when permitted, a connect user can select "I don't need to screen this patient" even if that patient doesn't have a BHI

    • reports for the screener should leverage the same conventions in the BHI sections that state "BHI was not generated as part of this screening encounter"

    • The BHI section will display an "N/A" in place of the BHI score and the referral drop down will show "select a referral option"

    • a comment is required before the user can proceed

In TRIDIUUM ONE (v3.57) DETAILED RELEASE NOTES | Tridiuum ONE Connect/Call Center (iFramed) , we introduced an iFramed solution where users do not need to see the patient search screen or other areas of Tridiuum after the screener is complete. As a result, we have added a new “Thank You” screen so the user understands the screener is complete and they can close the separate browser tab.

Please Note: This will only impact clients that have implemented this specific iFrame solution.
