Tridiuum ONE Connect Case Review

Case Review populates a queue of patients who have meet a specific criteria based on their assessment scoring known as a case trigger. When a patient triggers one of these case triggers, a case will be created. When a case trigger is generated, the provider will need to complete a Clinical Review Note for review by a Clinical Review Team Member. A notification will be alerted to the Clinical Review Team that a case has been created. The case is also added to a Case Review Queue so that the Clinical Review Team can monitor and follow up on the case.

Case Review Overview

Select Home Tab and select Case Review

Filter time frame to view case reviews that were generated in a specific time frame.

To search for a specific patient, enter the patients MRN or name into the search field to filter the queue.

To filter select the filter icon.

Once you have selected the desired filters, select apply to filter the list below.

Case Review can be exported to CSV.

Case Review displays patients name, triggers generated and the provider associated with the patient/assessment that generated the trigger.

To send a request to a provider to complete a Case Review Note, Select Request Review Note. This will notify the provider to complete the requested note. The date will display of when the request was sent to the provider.

Clinical Review Team can assign cases to specific Team members and update case status.

 

 

 

 

Case Review Triggers

Suicide: CSSRS >=3

BHI Flat Trend: stays the same for 4 consecutive assessments

BHI Declining: BHI (percentile or Tscore) gets worse for 4 consecutive assessments

BHI Subclinical Trend:

(SCAL) BHI 35 or less for 3 consecutive assessments

(NCAL) BHI 20 or less for 3 consecutive assessments

Treatment Plan Trend: Treatment plan alert triggered 3 times in a row

Therapeutic Alliance Trend: 3 consecutive 10 or under scores

Therapeutic Alliance Threshold: 5 total TA scores of 10 or under in episode of care

Extended Utilization: Patient reaches 20 sessions
