How to Find Patient Assessment PDF Reports (Call Center Screener, Intake, Update, and QA Reports)

The following training document will walk you through how to access patient reports to view, download, or print. There are two different ways to access the PDF Reports.

 

  1. On your Clinical Home, next to the patient who’s reports you’re interested in viewing, you can click on the date and the current assessment PDF reports will display along with with their QA Report.

  2. Find the patient who’s reports you want to access, and click on their name to open their Patient Profile Page.

  3. At the bottom of the Patient Profile Page, next to the assessment date you’re interested in looking for, click the green >.

  4. A drop-down to show the current PDF reports will appear. Select the report you’d like to view.
